Obsessive-compulsive Disorder (OCD)

Obsessive-compulsive disorder (OCD) is believed to be rooted in a biochemical imbalance of the brain. It is classified as an anxiety disorder that affects thoughts and actions.

It is characterised by recurrent, disturbing thoughts called obsessions that produce symptoms such as repetitive, ritualised behaviours called compulsions. While most patients experience both obsessions and compulsions, some experience only obsessions (about 20%), or only compulsions (about 10%).

A person with OCD may try to get rid of obsessions by engaging in compulsions, or by avoiding situational triggers. These obsessions may present as intrusive images or unwanted impulses. Most often, compulsions are practiced by sufferers as a way to alleviate anxiety. It is also common for compulsions to trigger anxiety when they become overly demanding.

A person with OCD may recognise that their thoughts and actions are pointless or excessive, but the drive can become so powerful that they have no control over the compulsion, despite knowing that it does not make sense.

Common types of obsessions and compulsions

OCD can manifest in many different ways, and everyone’s experience is unique. However, there are some common patterns that many people with OCD experience.

Common obsessions include:

  • Fear of contamination or germs
  • Intrusive thoughts about harm coming to yourself or others
  • Need for symmetry, order, or exactness
  • Unwanted sexual or violent thoughts
  • Religious or moral preoccupations
  • Fear of losing important items

Common compulsions include:

  • Excessive cleaning, handwashing, or showering
  • Checking behaviours (locks, appliances, switches)
  • Counting, tapping, or repeating actions
  • Arranging items in a particular way
  • Seeking reassurance from others
  • Mental rituals like praying or repeating phrases

It’s important to understand that having occasional intrusive thoughts or preferring things to be organised doesn’t mean you have OCD.

The difference is in the severity, frequency, and the level of distress these experiences cause in your daily life.

How OCD affects daily life

Living with OCD can be exhausting and isolating. The time spent on compulsions can take hours out of your day, affecting your work, relationships, and ability to enjoy activities you once loved.

You might find yourself:

  • Avoiding certain places or situations that trigger obsessions
  • Feeling embarrassed or ashamed about your thoughts or behaviours
  • Experiencing relationship difficulties because others don’t understand
  • Struggling with low self-esteem or feeling “crazy”
  • Developing depression alongside your OCD

Many people with OCD spend years suffering in silence before seeking help, often because they feel ashamed or don’t realise that effective treatment is available. Working with an OCD psychologist who understands the condition can provide the support you need.

How an OCD psychologist can help

The good news is that OCD is highly treatable with the right psychological approach. Working with a psychologist specialising in OCD can make a significant difference to your quality of life.

The most effective psychological treatment for OCD is a specific type of Cognitive Behavioural Therapy (CBT) called Exposure and Response Prevention (ERP). This involves gradually exposing yourself to situations that trigger obsessions whilst learning to resist the urge to perform compulsions.

Whilst this might sound frightening, ERP is done in a controlled, supportive way at your own pace. An OCD specialist psychologist will work with you to create a hierarchy of feared situations, starting with those that cause mild anxiety and gradually working up to more challenging ones.

Other therapeutic approaches that can be helpful include:

  • Acceptance and Commitment Therapy (ACT), which focuses on accepting intrusive thoughts rather than trying to eliminate them
  • Mindfulness techniques to help you observe thoughts without reacting to them
  • Cognitive restructuring to challenge unhelpful beliefs about intrusive thoughts

What does an OCD specialist psychologist do?

Psychologists specialising in OCD have specific training and experience in treating this condition. A clinical psychologist OCD specialist understands the unique challenges of OCD and knows how to tailor treatment to your specific symptoms.

During treatment, your OCD psychologist Sydney will:

  • Conduct a thorough assessment of your obsessions and compulsions
  • Help you understand the cycle that maintains your OCD
  • Develop a personalised treatment plan based on evidence-based approaches
  • Guide you through exposure exercises at a pace that feels manageable
  • Teach you skills to manage anxiety without relying on compulsions
  • Monitor your progress and adjust treatment as needed

The relationship between you and your psychologist is collaborative. You’re not expected to face your fears alone, and treatment is always tailored to what feels right for you.

Understanding the cycle of OCD

One of the key things you’ll learn when working with a psychologist specialising in OCD is how OCD maintains itself through a vicious cycle. It typically works like this:

  1. You experience an intrusive thought or obsession
  2. This triggers intense anxiety or discomfort
  3. You perform a compulsion to reduce the anxiety
  4. The anxiety temporarily decreases, which reinforces the behaviour
  5. The obsession returns, often stronger than before

Breaking this cycle is the key to recovery. Whilst it feels like compulsions are helping in the moment, they actually strengthen the OCD over time by teaching your brain that the obsessions are dangerous and need to be acted upon.

When to seek help from an OCD psychologist

If you’re wondering whether you need help, ask yourself these questions:

  • Do your thoughts or behaviours take up more than an hour of your day?
  • Do they cause significant distress or interfere with your life?
  • Do you feel unable to control them, even when you try?
  • Have others expressed concern about your behaviours?

If you answered yes to any of these, it’s worth speaking with an OCD psychologist who understands the condition. Early intervention can prevent OCD from worsening and help you regain control of your life more quickly.

You don’t have to live with the burden of OCD alone. With proper treatment and support from a qualified OCD psychologist Sydney, most people with OCD experience significant improvement in their symptoms and quality of life.

Getting started with OCD treatment

Taking the first step to address OCD can feel daunting, but it’s also incredibly courageous. Many people worry about what therapy will involve or whether they’ll be able to face their fears.

In your first session, we’ll take time to understand your specific experiences with OCD, how it’s affecting your life, and what you hope to achieve through therapy. We’ll work together to develop a treatment plan that feels manageable and is tailored to your needs.

Treatment is collaborative, and you’re always in control of the pace. Some people see improvements within a few weeks, whilst others need longer-term support. What matters is that you’re moving forward at a pace that works for you.

If you’re ready to take the next step, contact us to discuss how we can help. We also offer online therapy if that’s more convenient for you. You can learn more about our approach to therapy and counselling and view our fees and rebates information.
